We need your input!!

We are emailing the link to an online questionnaire to all LYSA members. You will be asked questions that help us to understand your perceptions of the services we provide. 

With your confidential response, we hope to gain insight on the current state of the association to help us evaluate current programs and to plan for the future. Our goal is to ensure playing soccer for LYSA is a great experience for all those involved in the years to come.
